Accessing Services

You will note from our staffing that we have a diverse work-force and your request for services may be managed by a staff member other than a Doctor.

You have a choice of either:

  • Using our online service 24/7 365 days a year to book certain appointments, request medication, viewing test results etc. You can request your online login details by telephoning us or by attending in person, however you will need to collect your online account details from the reception desk in person as we will need to verify your details for security purposes
  • Telephoning the Reception team on 023 9282 7132
  • Or you can call in to either practice and talk to our team

Please note that our Duty Team will triage and treat (diagnose and prescribe) a lot of ‘same day appointment requests’ that are not appropriate for the ‘Pharmacy Minor Ailments scheme’.

Our Outreach Matron will create and manage clinical care plans for the frail and housebound, educate patients on how to prevent any chronic diseases from escalating or deteriorating. She will also ensure that other health and care services are involved to ensure our patients are well-supported. Our Practice Pharmacist may be best placed to undertake your medication review and liaise with you in relation to your medications, dosages, potential drug interactions etc.

We can also book patients directly in to the Physiotherapy triage service provided by Solent NHS allowing you to quickly be assessed. You can also self refer by following this link: www.solent.nhs.uk/physioselfreferral

We will endeavor to complete your request at the earliest opportunity and this will depend on the nature of your request and its urgency. If your request means you need to see or speak to someone, appointments can often be booked up to 4 weeks in advance if your need is non-urgent/routine in nature. We offer face to face and telephone consultations to help meet our patients’ needs. If you wish to discuss more than one condition please book an appropriate number of 10 minute appointment slots.

Practice Extended Hours Clinics:

The business core hours are 8.00am – 6.30pm but we also hold ‘Extended Hours’ clinics outside of these times:

Monday:

7.00am – 8.00am
6.30pm – 7.00pm

Thursday:
6.30pm – 8.00pm

Friday:
7.00am – 8.00am

Saturday:
8.00am – 11.00am (usually first Saturday of the month):

We are available for both face to face & telephone/general/appointment enquirers during core hours of 8.00am – 6.30pm and additionally during those extended hours clinics.


Portsmouth Primary Care Alliance services:

Extended Access Services-

In addition to the practice’s own extended hours services, Portsmouth Primary Care Alliance (which is an organisation made up of GP Practices in Portsmouth) provides sustainable services to support General Practice and our individual practices and patients. Some of these services are shown below and how to access them; they are the:

Integrated Primary Care Service-

This is a new integrated 24/7 primary care service is set to improve care and patient access to GP services in Portsmouth. The Integrated Primary Care Service started on 29 June 2018 and incorporates the existing Acute Visiting Service and Extended Access Service and adds Out of Hours provision overnight. The NHS is linking these services to provide a ‘streamlined and unified’ new model for people needing urgent out-of-hospital care. You can now access appointments during evenings and weekends at a city-wide hub at Lake Road Health Centre in central Portsmouth. Additional routine appointments will be available as well as urgent same-day appointments every day until 10pm 365 days a year. To use the new service, during normal working hours call your usual GP surgery and at all other times call 111.

Acute Home Visiting (AVS)-

Since September 2015 the Alliance has provided an Acute Visiting Service. This now runs during the hours of 9am-6pm Monday – Friday (excluding Bank Holidays). If you are feeling unwell and are not able to attend your usual doctor’s surgery, you can contact your GP Practice to request a Home Visit. Some patients will be seen by their usual GPs but if following a telephone assessment your GP feels that you are unable to wait until after surgery for a visit, you may be offered a referral to the Acute Visiting Service. One of our team of GPs will then come out to see you as a representative of your practice. Our team of doctors work across the Portsmouth area to visit registered patients at home. They provide appropriate treatment which helps to both reduce attendances at A&E and avoid unnecessary admission to hospital. Patients referred to the AVS are normally seen quickly (normally within 2 hours of us receiving the referral) and are given the best care in the most appropriate setting.

Online Services:

SystmOnline is the mobile app that allows you 24/7 access to a number of services that would traditionally require a phone call or visit to the practice. The application can be used on a PC, Android or iOS operating systems, although some features are not fully compatible with iOS.

Once you have the app you will need a username and password, which we will supply you in person, enabling you to access the following services:

  • Requesting Medication (repeat only)
  • Booking Certain Appointments
  • Viewing test results, along with comments from the GP
  • Complete Travel Questionnaires online saving you time when preparing for your trips abroad
  • View your medical record, either a full coded record or a detailed record from 1st July 2016 (in accordance to NHS guidelines)
  • Do all of the above but for other members of your household by means of proxy access. For example parents can have access to their children's record, or where consent is given, access to an elderly relatives record, giving them all the advantages of online access without needing to be an IT whiz.

MJOG - SMS Messenging service

We, and a number of other practices in the Portsmouth area, have introduced a new way of contacting our patients with details of appointments and other reminders. Up until now you have not been able to respond to these messages other than calling us on the phone. but now with MJOG you will start to see the option to reply to these messages, hopefully saving you time.

If you have a smart phone you can install the MJOG Messenger application from the Android and IOS app stores which will allow you to keep a record of upcoming appointments as well as being able to respond to invites at the push of a button.

The messenger application will compliment the existing use of SMS, Email, Phone and Postal methods of keeping in touch with you.

It is important that we hold your mobile phone number on our records as this method of contact will be used increasingly in delivering a service and managing your care, although you of course have the right to Opt Out of being contacted by SMS / Email by letting us know in writing.

Zero Tolerance Policy

Abusive or Violent Behavior

Please note that the East Shore Partnership operates a “Zero Tolerance” policy. This is where a patient commits an act of violence or aggression against a member of staff or other patient, or behaves in such a way that a person fears for their safety.

Any such incidents may be reported to the police, the patient’s treatment will be terminated and if applicable the local authority will be informed.

Expected Standards of Behavior

The East Shore Partnership has a duty to provide a safe and secure environment for staff, patients and visitors. Violent or abusive behavior will not be tolerated and decisive action will be taken to protect patients and staff.

The following are examples of unacceptable behavior on Practice Premises:

  • Violence
  • Excessive noise, e.g. recurrent loud or intrusive conversation or shouting
  • Threatening or abusive language involving swearing or offensive remarks
  • Derogatory, racial or sexual remarks
  • Malicious allegations relating to members of staff, other patients or visitors
  • Taking alcohol or drugs on practice premises
  • Drug dealing on practice premises
  • Willful damage to practice property or theft
  • Threats or threatening behavior.
